I consent to allowing Jumble & Sense to collect and store the confidential information I have emailed to them pertaining to my child (report cards, Individualized Program Plans, professional assessments) for one calendar year from the date on this form. After a year, all personal information pertaining to my child will be deleted from their files.

I consent to Jumble & Sense communicating with, obtaining information from, and sharing relevant information regarding my child with professionals, service providers, educational staff, healthcare practitioners, and other individuals or organizations involved in, or consulted for, the purpose of supporting my child's needs, development and well being.
